  File is a zip file containing the Excel file. As required by the Rules and Regulations (Rule 203) you need WinZip or an equivalent on your PC. Game Report must be printed in landscape mode.

Game Evaluation Form The game evaluation form can be obtained by going to your division and clicking on the game number.

Call-Up Form

  Form must be printed in portrait mode.

Referee Travel Reimbursement Form (PDF)
For use by referees that officiate RDYSL games and travel more than 30 miles.  This is the only form that will be accepted.

Player Medical Form
This medical form is provided as a guideline in the hopes that someday there could be a standardized medical form used by all area tournaments.  It is the desire (but not a requirement) that area tournaments would accept this medical form as a suitable replacement for their own tournament medical form.
